The basic Data Model will follow this Tempalte :-

I am in the UK< but if you are in the US, the data will look very different, and will likely have an emphasis on Costs on Treatment, Items used, Insurance Claims and so on.
If you like to give me some details of the data that you want to store and analyse in the Data Mart, then we can move ahead.
